PHA Concern Follow-up and Resolution

Process Hazard Analysis (PHA) normally involves the use of qualitative techniques to identify and assess hazards associated with deviation from design intent for safety significance.

There are different PHA techniques utilized such as HAZOP, What If Analysis, Failure Modes and Effects Analysis and LOPA. These PHA techniques typically result in findings and recommendations related to overpressure protection that require follow up by qualified engineers in relief system design.
